The primary goal of the Viral Hepatitis (VH) Services program is to increase the proportion of persons in Indiana who are aware of their HCV infection and linked to care by assisting individuals living with Hepatitis C linking to and establishing care; ensuring continuity of care; and promoting self-sufficiency through customized education, coordination of services and empowerment of the individual.

The objectives of the Viral Hepatitis Services Program are:

  • To promote a single point of entry to a variety of needed services;
  • To develop a plan of action with an individual, based on desired outcomes and needs (i.e., treatment, etc.);
  • To manage referrals to ensure success;
  • To identify gaps in services and leverage community resources to address those gaps;
  • To provide customized education and support to an individual through identified actions; and
  • To coordinate and review an individual’s plan of action, and work with them to make modifications, as necessary.
